Subacute/Transitional Care
What is Subacute Care?
Subacute care usually refers to the temporary interval following a hospitalization or pre-stabilization period of a chronic condition before an individual is well enough to go home or transfer to a more appropriate program unit.
In the past, when an individual had sufficiently recovered from an acute episode and no longer needed to receive highly complex medical care -- following, for example, a stroke or heart or orthopedic surgery -- they were transferred to a lower-cost hospital unit to continue their recuperation.
Now there is an alternative. The Botsford Health Care Continuum offers Subacute services in a first-class, skilled-nursing setting at Botsford Rehabilitation and Continuing Care Center (BRACC). Clients are referred to BRACC's Subacute program by their physician, directly from a hospital or other setting.
Who Pays for Subacute Care?
Subacute care is covered by most third-party insurances, managed care programs, and Medicare.
What Services are Offered in BRACC's Subacute Program?
Services include complex medical/nursing, post-operative recovery, wound care, orthopedic care, closed-head injury care, rehabilitation, hospice care, respiratory or pulmonary management, and cardiology management.
What Role Does a Client's Primary Care Physician Play in the Subacute Program?
The program is under the supervision of a clinical director who is a qualified and experienced physician. The clinical director is in consultation with the client's primary physician to monitor and implement the specific medical or rehabilitation protocols prescribed for the individual.
