Today, more than ever, individuals are living to an advanced age or living with irreversible chronic conditions. The nature of many of these conditions is that the individuals diagnosed with them will require increasing levels of care over time.

When the time comes that care needs exceed the services able to be safely and regularly provided in the current home setting, Botsford Rehabilitation and Continuing Care Center's Chronic Care program is there for you.

Within the realities of their chronic condition, our residents enjoy a wealth of experiences and are encouraged to broaden their world. Not only are they provided 24-hour skilled nursing services - they are offered total and supportive long-term care.

At Botsford, it's not just what we do...it's how we do it. We call it Person-Centered Care. When your condition allows you control over so little of your life, let us help you take it back. Have the morning newspaper with a made-to-order omelette. Bake some muffins. Have your family over for dinner and take over a kitchen for a private family dinner. Email your friends and family from our computer lab. Or curl up with a good book. There are hundreds of selections in our Library.

There will be days when you just want to get out and get around - get some fresh air. Our beautifully landscaped campus is wheelchair accessible. Take in the sun in the Linda S. Mlynarek Community Garden. Or enjoy an outing to an area park or theatre on the Botsford Commons Senior Community bus.
