Assisted Living - Discharge Criteria

Botsford Assisted Living Center is dedicated to providing care to those individuals who require a minimal amount of assistance with activities of daily living. For this reason, clearly defined discharge criteria are necessary to ensure a safe environment for each of the residents of Botsford Assisted Living Center.

The resident's ability to safely remain in the Assisted Living Center will be re-evaluated by the nurse at least quarterly, and more often if there is a significant change in the resident's physical, cognitive or behavioral condition.

The discharge criteria includes several specific categories and each is clearly defined as follows:

Combativeness
Any combative behavior which is a threat to self or others or becomes a disturbance to other residents.

Dementia
Symptoms of dementia exceed manageability For example; wandering or behaviors that hinder self, residents and/or staff.

Immobility
A resident can no longer ambulate and transfer with the use of assistive devices. Their condition requires a two-person transfer.

Intervention
Skilled nursing interventions are needed 24-hours a day for an extended period of time.

Medication Reminders
A resident can no longer safely self-medicate. Residents must be in compliance with the Assisted Living Center Medication Reminder Policy.

Toileting
A resident cannot or will not participate in managing incontinence.
