Assisted Living - Admission Criteria
Botsford Continuing Care Corporation's Assisted Living Center is based on a SOCIAL MODEL and is dedicated to emphasizing "wellness" among its residents by striving to ensure our residents maintain and retain their independence as long as possible.
Our admission criteria is designed to facilitate each resident's needs and encourage each resident to actively participate in the decision-making process.
The admission criteria includes several specific categories and each is clearly defined as follows:
Pre-Admission Nursing Assessment
A pre-admission nursing assessment is completed at the time of application to determine the resident's needs. The nursing assessment evaluates physical, cognitive and behavioral conditions and is intended to prevent any conditions which may create an unsafe environment for any resident living in the Botsford Assisted Living Center.
Dressing
Residents may require assistance with dressing and in selection of clothing. Residents should be able to assist with task.
Hygiene
Residents require minimal assistance with bathing, grooming and hygiene. They need reminders, set-up, and assistance with preparation. Residents should be able to assist with task.
Medication Reminders
Medication reminders and monitoring will be provided to all residents. A detailed description of the medication reminder guidelines is included in the Assisted Living Center Medication Policy and is provided to each applicant and/or family member(s) at the time of the pre-admission nursing assessment.
Mobility
Residents must be mobile or able to ambulate and transfer with the use of assistive devices. Residents may require occasional assistance with ambulating and transferring from wheelchair to chair, bed or toilet.
Toileting
Residents must be continent or able to manage incontinence independently or with a minimal amount of assistance.
